A two part predicate that measures how long an event has been going on, requiring a phrase of quantity and locating the end of the measurement at the tense.

The construction is the only member of the gerund family with an obligatory third element, and dropping the quantity phrase makes it ungrammatical rather than merely vague. What it measures is the stretch from the start of the event to the point the tense locates, so the present tense measures up to now and the imperfect measures up to a past reference point. It competes directly with the pattern built on the impersonal verb of time and with the simple present accompanied by a phrase meaning since, and the three differ in what they make the subject of the clause. Under negation it measures the stretch during which the event has not happened, which is the same measurement with the polarity of the gerund reversed.
